Akizmet is a plug-in that comes with WordPress.   Like some other features you have the choice of using it or not.   It is highly recommended you do.

The only pre-requisite is to get an API key by registering at wordpress.com

You don’t need to establish a wordpress.com site unless you want to;  but either way you  need to be a registered user there to get your API key.

As far as I know, the API key can be used with any of your wordpress based sites  ie. no need for a different API key for each site.
